Anamnesis

What is an anamnesis?

When you go to the doctor, a medical history is taken. Anamnesis means that the doctor takes important information from you. We also say: your medical history is taken.

This involves information that is important for the treatment.

The medical history is usually taken during a consultation with the doctor. In preparation, you often have to fill out a medical history form beforehand. The doctor will then already know some of the information before the consultation.

It is important that you answer all questions honestly. The doctor wants to understand your complaints exactly. This is the only way he can treat you properly.

It is important that you have confidence in the doctor. Some things are uncomfortable to say. Nevertheless, be honest.

There are many different areas that are important when taking a medical history.

The doctor will ask questions about your medical history. For example, about previous illnesses or operations.

They will also ask about allergies or previous chronic illnesses. This is called a personal medical history.

Some diseases are genetic. Genetic means that diseases occur several times in a family.

This is why the doctor asks about illnesses in the family. Then he will know better whether you also have the disease or could get it in the future. This is called a family history.

The doctor will often also ask you what medication you are taking. Also tell the doctor if you only take some medications occasionally.

The doctor will also ask if you drink alcohol, take drugs or smoke. This can trigger or aggravate certain illnesses.

In general, your lifestyle is important. For example, whether you get enough sleep or exercise.

Your job can also be important. Or your family situation. Certain situations can cause stress. This often makes you ill.

It is important to know that the doctor is bound to confidentiality. So you can say everything openly. The doctor is not allowed to tell anyone else.

What is a medical history form?

The medical history form is a form that you often have to fill in at the doctor. Especially when you visit the doctor for the first time.

You will usually be given the form at reception in the doctor’s surgery. You will then have time in the waiting room to complete the form.

It is important that you complete the form honestly. Correct and complete information will help the doctor to treat you correctly.

You must enter personal details on the medical history form. For example, your name and address. Or your health insurance company.

In most cases, you will also need to enter information about your health. For example, whether you are taking medication or have any illnesses.

A medical history form is different in every practice. Sometimes one doctor asks for certain data that another doctor does not need.

For example, there are specialists who need specific information about your health.
